ALL PAPERWORK REQUIRED PRIOR TO THE BEGINNING OF PRACTICE

 

  1. Notification of RISK: Signature of parent or guardian.

 

  1. Medical History Evaluation: Signature of parent of guardian and signature of physician is required. Physical examination with signature of a physician is required. (Sports Participation Packet- Physicals are valid for one year from the date of previous physical)

 

  1. Contract of Parents or Guardian consent, Waiver and Indemnification: Must be signed by parent/guardian and athlete and MUST BE NOTARIZED!

 

  1.  Insurance information: Must designate school insurance or home policy with the name of  

              the insuring company and policy number.

 

  1. Emergency Treatment Form: A copy of this completed form must accompany all athletes while on away trips. It is also good practice for all coaches to keep a copy available at practice in the event a player is injured.

ATHLETIC PARTICIPATION

 

1.    Students are not allowed to participate in practice or games while that student is assigned to ISS or out-of-school suspension.

 

2.     No student is allowed to try out, practice, or participate in a sport without first having completed an emergency treatment authorization form and sports participation packet with proof of insurance on file in the AD’s office and principal’s office.

 

 

 

3.       Coaches should keep a copy of the completed emergency treatment authorization forms with him/her at all times in the event a student is injured and requires medical treatment. 

 

4.       Eighth grade players MAY NOT PLAY ON A JUNIOR VARSITY TEAM AND THEN DROP TO A SEVENTH/EIGHTH GRADE TEAM AT THE END OF A SEASON.

 

ACADEMIC ELIGIBILITY FOR EXTRACURRICULAR ACTIVITIES

 

  1. Starting with the 2005-2006 school year, the first nine weeks eligibility will be based on the grade point average for the LAST MARKING PERIOD of the previous school year.

  2. Please remember, the 2.0 GPA is still the average the student must attain, the only thing changed is the time factor.

  3. Students must be present at least 4 periods of a school day or have a pre-arranged absence on the day of an event to participate in a school activity.

  4. A middle school student may participate in interscholastic athletic competition until  he/she reaches the age of FIFTEEN YEARS AND NINE MONTHS. Students NOT academically promoted are ineligible to participate. Participation in middle school is limited to seventh and eighth grade students only. (Frostproof is granted a waiver for this rule allowing their sixth graders to participate in football).
